Transcripts
All students who take courses from American Heritage School may request a transcript for themselves or for another educational institution.
One transcript per student will be provided free of charge. Additional copies will cost 2 dollars each, payable by check or credit card to American Heritage School.
Transcript requests must include a Request for Transcripts form (pdf form). Therefore, requests and payment will not be taken over the phone. This request form and payment information or check must be faxed to 801-642-0060 or mailed to:
